body

BACKGROUND INFORMATION:

The Lodi Fire Department (“Department”) has contracted with Lodi Memorial Hospital Association, Inc. (“Lodi Memorial”) for firefighter physicals since 2013. These physicals are required per the Lodi Professional Firefighters Memorandum of Understanding and recommended by NFPA 1582/1583. Over the past 10 years, Lodi Memorial has been a cost effective and practical solution for the Department by reducing overtime costs, because personnel were able to complete their physicals in Lodi while on duty.

Despite these benefits, the service levels provided by Lodi Memorial have declined and the 2023 physicals did not meet the contract obligations, nor provide all of the required services. Primary issues included scheduling errors, lack of confirmation or follow-up by Lodi Memorial, and some Department employees had to go to Stockton to complete their physicals, which resulted in unexpected overtime payments to staff. Due to these challenges, the Department will be amending the existing contract with Lodi Memorial, to reduce the scope of the contract to only annual vaccination requirements for Department personnel.

In light of the upcoming amendment with Lodi Memorial, the Department solicited quotes from four vendors for on-site physical services and four proposals were received:

Life Extensions Clinics Inc.

$57,278

Professional Health Services

$61,400

1582

$66,820 - does not provide blood draw on site as other vendors do

Health Fit Clinic and Selectwell Services

$67,080

Pricing for the two lowest vendor proposals was comparable; therefore, references were contacted to evaluate performance and satisfaction levels with the different on-site physical providers. Ultimately, Life Extensions provided the lowest bid and its reference check showed that it services many neighboring fire departments and has outstanding satisfaction levels. Life Extensions proposed services include on-site bloodwork, the required firefighter physicals, and ultrasounds to personnel for the NFPA recommended cancer screening.

Staff recommends authorizing Interim City Manager to execute a Professional Service Agreement with Life Extensions for one year with two, one-year options to extend, for a total amount not to exceed $180,000 over the course of the maximum three-year term.
