Adopt a resolution authorizing the Interim City Manager or Designee to execute a lease agreement in a form acceptable by the City Attorney with the County of San Joaquin for spaces identified for SJ Health within 710 N Sacramento Street.

BACKGROUND INFORMATION:
In late 2024, the City of Lodi began collaborating with San Joaquin County (SJC) and SJ Health to explore an expanded partnership to support the region’s most vulnerable populations through transitional housing and behavioral health stabilization services. This partnership represents a strategic realignment of resources to better address homelessness, mental health crises, and housing insecurity.
As part of this effort, the County has proposed to:
• Implement behavioral health services within the Lodi Access Center, including a mental health quiet ward and behavioral health respite beds.
• Implement operation of a County medical clinic and related public health and health center services within the Access Center, including associated administrative, clinical, and support functions
San Joaquin County has identified approximately 6,000 square feet of the Access Center (includes lobby space), to be designated for a clinic and behavioral health services, including a mental health quiet ward and behavioral health respite beds.
Last year, SJC and the City executed the infrastructure agreement for the behavioral health spaces and are now ready to execute the lease with SJ Health so they can begin their infrastructure project for the clinic space.
Proposed Service Components within 710 North Sacramento Street for the SJ Health Clinic will do all of the following:
• Be operated by San Joaquin Health Centers as a medical clinic providing public health and health center services in accordance with the lease agreement.
• Utilize approximately 2,757 square feet of leased space to support clinical, administrative, and patient service functions.
• Deliver accessible, community-based healthcare services, including primary care, preventative services, and coordination of behavioral health supports.
• Serve individuals experiencing homelessness and other vulnerable populations within the community, with an emphasis on reducing barriers to care.
• Enhance system coordination by co-locating or closely aligning services with broader City and County efforts to address homelessness and public health needs.
COMMUNITY BENEFITS:
This proposed clinic strengthens the City and County’s shared commitment to expanding access to healthcare and improving outcomes for vulnerable populations. Key benefits to the Lodi community include:
• Expanded Access to Care: Residents, including individuals experiencing homelessness, will have access to consistent, community-based medical services, reducing barriers such as transportation, cost, and system navigation.
• Improved Health Outcomes: Early intervention, preventative care, and chronic disease management help reduce the severity of health conditions and improve overall community wellness.
• Reduced Strain on Emergency Services: Providing appropriate, non-emergency medical care decreases reliance on emergency rooms and urgent care systems for routine or preventable health needs.
• Integrated Continuum of Care: The clinic supports coordination between healthcare, behavioral health, and housing resources, creating more seamless pathways from crisis to stability.
• Support for Vulnerable Populations: Targeted services for high-need individuals, improve care continuity, reduce gaps in service delivery, and promote long-term stability.
• Enhanced Community Well-Being: Increasing access to healthcare contributes to a healthier, more stable community, supporting overall livability and quality of life in Lodi.
FISCAL IMPACT:
The proposed agreements with San Joaquin County for the SJ Health Clinic will result in the City of Lodi receiving $3,500 monthly for lease payments beginning in FY27 and has been included in the FY27 budget to be presented at a future date. All future-year budgets will incorporate ongoing lease revenues as applicable through the lease term.
